2. Prize choice (first, second, and third choice)*

3. Address to mail the prize to

*The Fine Print: Prize choice will be a first come first choice basis. If the number of Hide & Seek winner entries exceeds the number of prizes, a drawing will be held and the prize winners selected at random if matching up 1st, 2nd, and 3rd choices are duplicated.
